Integrated Supportive Living (ISL) will provide adults experiencing disability at home and the daily supports needed to thrive as independently as possible. The home will be shared among two participants, each with their own private bedroom space and shared common living area. The ISL homes are fully accessible if needed, ensuring a safe and comfortable environment tailored to the unique needs of all participants. Individuals will stay for short or medium lengths while they learn the skills to live independently.
This living environment will provide holistic and collaborative support to individuals so they can maintain autonomy and control over their lives, living as independently as possible. ISL will provide personalized support and services tailored to meet the needs of each participant, ensuring their success. Clinical needs will be addressed as needed, and supports such as nursing, psychiatry, psychology, occupational therapy, speech and language therapy, and music therapy may be offered to support the physical, spiritual, emotional, and social well-being of the individual.

Each ISL home operates in a person-centred, strength-based, affirming, non-restrictive, and inclusive environment that is warm, supportive, and empowering. Our intention is for all individuals experiencing disability to feel valued and respected and to have a deep sense of belonging and connection. Everyone’s unique strengths and abilities are celebrated, and their happiness and well-being are at the centre of everything we do. We empower our participants to live their best life through individualized person-centred care and support, meaningful activities, skill building opportunities, and a strong sense of community.
Who we support
We support adults experiencing disability who wish to live in their own home as independently as possible. ISL services are designed for individuals with less intensive behavioural and/or medical support needs who require a lower level of care than what is provided in a supportive living program.
As part of the pre-admission assessment, a meeting will be held with the prospective participant, their primary clinician, and the referring case manager. During this meeting, the participant’s goals, aspirations, and desired outcomes will be explored. The Supports Intensity Scale – Adult Version (SIS-A), a standardized tool designed to assess the type and level of support needed for success in a community-based setting, will also be completed.
If all parties agree that placement in ISL aligns with the individual’s person-centred plan and will support them in achieving their goals, additional planning and coordination for admission will proceed.
